The Six Nations Primary Care Team is a diverse multidisciplinary team of practitioners of Haudenosaunee and Western medicine practices. Our mission is to build and maintain a Healthy Community through patient-centered, wholistic and integrated care.

Provide integrated care through a dynamic team of caring healers and Onkwehón:we utilizing Western and alternative medicine to effectively nurture health and wellness.

Mission

We recognize and respect the parallels of Onkwehón:we and Western medicines in the spirit of the Two Row Wampum. With this recognition we help individuals, families, and the community journey along their pathway to optimal mental, physical, emotional and spiritual wellness.

Our Team and Services Offered:

Our model utilizes Integrated Wellness Teams, also known as “Pods”.

Each ‘Pod’ consists of a:

  • Family Doctor
  • Shared Nurse Practitioners
  • Nurse Case Manager
  • Registered Practical Nurse
  • Medical Administrative Clerk
  • Shared Cultural Advisor

 

Integrated Services / Clinics

As part of the integrated care model, there will be services that are available to all community members. These include the following:

  • Integrated Support Team members accessible through self-referral
    • Dietitian
    • Traditional Medicine Healer
    • Indigenous Midwife
  • Integrated Wellness Clinics accessible through self-referral
    • Internal Medicine Rapid Assessment Clinic (IMRAC) - provides improved access to internal medicine consultation for patients who have highly complex and complicated medical conditions and support connections to other specialists as appropriate.

Intake/Referral Process:

Please complete the following intake form and either

  • Fax to: 519-445-1917, or;

  • Email to: snfht@sixnations.ca or;

  • Drop off to the Family Health Team located at White Pines Wellness Centre, 1745 Chiefswood Road, Ohsweken ON 

Once we receive your intake form, you will be added to our waitlist. We prioritize patients based on their medical conditions, similar to the triage system in an Emergency Room. This ensures that those with urgent needs are seen first, while also considering the length of time others have been waiting.

Six Nations Health Services in collaboration with St. Joseph's Hospital of Hamilton, is pleased to announce the opening of an Internal Medicine Rapid Assessment clinic (IMRAC) on Six Nations territory.

The clinic will have the capacity to receive referrals from healthcare providers and community partners who serve Six Nations community members and those identifying as Indigenous in the area.

The clinic will offer rapid internal medicine assessments locally. This will eliminate the necessity for community members to travel elsewhere for services.

The clinic will see Indigenous patients with:

  • Chronic complex medical conditions
  • Uncontrolled diabetes
  • Hypertension
  • COPD
  • Not Yet Diagnosed (NYD) acute medical conditions that are stable enough to not needed the emergency room.
  • Common Internal Medicine conditions
  • Common internal medicine procedures- skin biopsies, thoracentesis, paracentesis, diagnostic lumbar puncture, joint injections/aspirations.

Patient care is delivered in a culturally safe manner, with the option for patients to incorporate traditional healing practices and allied health services into their care plan.

Referrals may be sent via fax to the Primary Care Management Team at Six Nations Health Services at 519-445-4679. A physician will then assess and prioritize these referrals according to the patient's level of urgency, ensuring that they are seen promptly. In cases where a clinic is scheduled on the same day, we will make an effort to triage and accommodate same-day patient referrals when necessary.
