Mental Health and Addictions Services

We provide a number of mental health and addiction services for members and residents of Six Nations of the Grand River Territory. Our purpose is to support our clients who are experiencing issues with substance use and/or mental illness. The type of support varies based on the needs of the individual. It is important to us to promote awareness, education and understanding about how mental health and substance use affect the individual, families and our community.

Intake/Referral Process:

Individuals can be referred by family, doctors, hospitals, police, service providers or individuals may refer themselves. However, the individual MUST consent to the referral.  Client intake is completely voluntary.

Children and youth must be referred by a family doctor.

Please call 519-445-2143 to set up an appointment with a case manager.  Once you have an appointment set, please bring your health card, status card and any current medications you are taking to your first appointment.

Your first visit will provide an overview of services, a holistic mental health assessment and signing of relevant consent forms.

Program Description:

We provide case management for Six Nations members seeking assistance with mental health and/or addictions issues.  Psychotherapy/counselling is NOT provided, however referrals to appropriate providers are provided if necessary.

Program Description:

We have a limited number of 'geared to income' housing units for adults (18+) experiencing serious mental illnesses or addiction, and are homeless or at high risk for becoming homeless.  We do not accept clients with a criminal record of violent assault or crimes against children.

Clients are able to sign a one-year lease, which is renewable each year.  Tenants will need to follow a code of conduct which restricts usage of drugs/alcohol on the premises.

We have units located in Ohswé:ken, Brantford, Hamilton and Niagara.
