Intake/Referral Process:

You may refer yourself or you may be referred by others such as family members, friends, neighbours, doctors and hospitals.

1.      Contact Home & Community Care Case Management Office at 519-445-0077 for intake to any of the HCC programs/services

2.      You will be asked to provide your name, date of birth, 10-digit status card number, health card number, address and phone number

3.      You will be assigned to a Case Manager who will call you to set up an assessment

4.      After your assessment, the Case Manager will send the necessary documents to requested service providers

5.      You will be contacted by the service providers to arrange initial assessments
