Program Description:

The Six Nations Child and Youth Health team is here to support you and your child with diagnosed or suspected Autism Spectrum Disorder (ASD). As a family-centered, strengths-based program we respect and value your priorities and goals and we focus on the strengths of your child.  Our team will meet with you to understand your goals for your child and work with you to develop a plan to meet those goals.

Services Offered:

  • Support in accessing services  

  • Support in seeking diagnosis

  • Direct treatment supports by our interdisciplinary team including Speech Language Pathology, Occupational Therapy, and Board Certified Behaviour Analyst 

  • Delivery of Ontario Autism Program (OAP). Through a partnership with Hamilton Health Sciences, we are now able to offer the following OAP services on Six Nations:

Social ABCs is a 12-week Caregiver Mediated Early Years program in which parents work with a trained coach from Child and Youth Health Team to learn strategies to help your child improve communication and sharing of positive emotions with others.

Eligibility:

  • Children diagnosed with Autism Spectrum Disorder between the ages of 12 to 42 months (3 and a half years) at the start of treatment…, and

  • Have social communication difficulties, and

  • Are registered with the OAP and

  • Received an invitation letter for this service from the Ministry of Children, Community and Social Services (MCCSS)

Entry to School (ETS) is a six-month curriculum-based program to help your child get ready to attend school. Our providers will work with you to develop an individualized program with virtual, in-person, and group-based options to best serve the needs of your child and your family. The ETS program is followed by transition supports and consultation for up to the first six months of school.

Eligibility:

  • Children diagnosed with Autism Spectrum Disorder who are entering school for the first time (kindergarten or grade 1), and

  • Are registered with the OAP and

  • Received an invitation letter for this service from the Ministry of Children, Community and Social Services (MCCSS)

Program Description:

Provides support to families in navigating services for their children with health, social and or developmental needs.

Services Offered:

  • Collaborates with families to complete a needs assessment

  • Supports in advocacy in the best interest of the child and family

  • Works with families to identify their priorities values and goals

  • Coordinate services and supports within and outside the community

Program Description:

Provides support for children and their families with a child diagnosed or with suspected diagnosis of Fetal Alcohol Spectrum disorder

Services Offered:

  • Provides education about Fetal Alcohol Spectrum disorder

  • Offers a safe space for families to share their experiences and strengths 

  • Facilitates and coordinates caregiver support groups

  • Supports diagnostic assessment if interested

  • Organizes events around awareness and destigmatizing FASD in the community

Program Description:

To support the nutritional needs of children, youth, and their families by applying all aspects of the nutrition care process

Services Offered:

  • Delivery of innovative, evidence-based food and nutrition programming for clients aged 0-18

  • Nutritional assessments

  • Assistance with food access

Program Description:

Delivers Early Childhood Development Services in Six Nations Elementary schools and provides support in 0-6 in succeeding with transitional periods throughout this age group.

Services Offered:

  • Provides individual and group programming for children in JK to Grade 2 across Six Nations Elementary Schools.

  • Provides program delivery for 0-6 and their families in partnership with other programs under the Child and Youth Health portfolio

Program Description:

Provides In school health curriculum education and support in collaboration with other health partners.

Services Offered:

  • Supports in teaching the Health curriculum education for JK to Grade 8

  • Provides support for school staff on health needs for children

  • Promotes healthy lifestyle

  • Provides First Aid support at school events

Program Description:

Provides support for families to develop skills in daily life activities at home in school and in the community.

Services Offered:

  • Supports development in areas such as dressing, eating, sleeping, toileting, classroom participation and play

  • Life skills such as budgeting, homework, planning, self care and sports

Program Description:

Supports children & youth and their families to connect with services, cultural/traditional knowledge and opportunities in their communities to improve child & youth outcomes

Services Offered:

  • Provides cultural teachings through group programming

  • Provides family consultations

  • Offers programming in the community for children and youth

  • Builds bridges between Health Services and other community partners and schools

Program Description:

Provides services in promoting wellness, self-management strategies, to assist children, youth, and their families in their wellness journeys through therapeutic intervention

  • Offers community programming to build confidence and resilience

Program Description:

Supports children 0-18 with identified speech and language needs in developing and improving their overall ability to communicate.

Services Offered:

  • Our providers work with families to assess Speech and language needs

  • Support children and youth through direct sessions or collaborative consultations based on needs

  • Consultations take place with schools, daycares and community partners
