• Palliative Case Management/Intake/Assessment

• Care-planning as well as Advanced Care Planning

• Respite and Caregiver support

• Psychosocial/emotional/spiritual support as patients and families experience life-limiting illness

• Grief and bereavement support

• Pain and symptom management

• Volunteer Onboarding/Training for friendly visiting

• Referrals for additional services as needed (Hospice, MOW, Medical Transportation, Traditional Medicine, Child & Youth, Mental Health)

• 24 hr-outreach nursing support for End of Life support(PCOT)

• Specialized Palliative Personal Support Services
